MedWise™ is a unique software platform offered by Tabula Rasa HealthCare (TRHC) that identifies simultaneous, accumulative, multi-drug interactions to help ensure medication safety, efficacy, and adherence. Interoperable with athenahealth, MedWise provides clinicians with access to TRHC’s proprietary technology to aid in the reduction of preventable adverse drug events (ADEs), thereby improving quality, enhancing outcomes, and reducing total costs. MedWise adds value to existing technology investments through its detection of multi-drug interactions and advanced analyses.

MedWise helps providers optimize medication regimens by calculating patients’ MedWise Risk Scores™ at the point of prescribing. MedWise improves workflow by highlighting only scores requiring attention. Risks identified are actionable and documented to improve outcomes and medical cost savings.

In a survey of physician users, more than 80% responded that TRHC's MedWise Risk Score helps them provide better care, and more than 90% would use it again and would recommend it to other prescribers. Upon prescriber request, TRHC's board-certified clinical pharmacists are available for prescriber consultation. Patients under the care of prescribers using MedWise have experienced downstream medical cost savings approaching $400 per patient per month.

MedWise helps value-based care providers prevent ADEs using decision support tools with pharmacists available to help optimize medication regimens. Provider organizations that take a financial risk, including those within ACOs or other value-based care arrangements, would benefit most from the use of MedWise.

Product Features - SMART APP Core Software that includes TRHC’s MedWise Risk Score, a set of risk factors, bullseye graph identifying the source of the risk, TRHC’s proprietary Medication Risk Mitigation Matrix™, and 4Viz visualization dashboard - Access to MedWise PharmD-level support - CME, CNE, and ACPE-accredited clinical training and education - Product implementation and orientation services

Contact TRHC for more information and to request Medication Safety Reviews™ by board-certified pharmacists, and for PGx services or SaaS licensing of MedWise to in-house pharmacists.

How MedWise Medication Risk Mitigation Works Clinicians are able to analyze both pharmacokinetic and pharmacodynamic risks, including accumulative multi-drug and drug-gene interactions. TRHC’s Medication Risk Mitigation Matrix delivers point-of-prescribing medication decision support, including precision dosing systems that are documented to improve outcomes, and presents actionable opportunities using the MedWise Risk Score for risk mitigation.

Workflow Benefits TRHC’s SMART on FHIR integration improves and enhances existing clinical workflow. Medication decision support tools are available at the point of care and a click away, enabling clinicians to:

  • Manage and optimize patients' medication regimens
  • Access TRHC’s science through a variety of clinical and educational platforms
  • Focus on patients’ medication risk factors to personalize medication regimens and reduce risk rather focusing on one-to-one drug interactivity
  • Use the decision support sandbox to conceptually modify patients’ medication therapies in simulation, while understanding the change in medication risk using the pre-and post-MedWise Risk Score
  • Use the MedWise Risk Score as a performance marker to help identify and reduce preventable ADEs
  • Consult a board-certified clinical pharmacist for a Medication Safety Review, including physician recommendations, a patient action plan, and a patient time-of-day administration profile

For opioid management, MedWise is paramount because it identifies risks for dangerous accumulative side effects, unintentional overdose, and ineffective medication regimens.
